• To ‘Whom It May Concern’ letters – £20.00

Reports/Medical Examinations

• Employers Report and Opinion with/without Examination – £110.00
• Taxi Medical Examinations and report – £110.00
• Medical Reports for e.g: mortgage/ life insurance etc – £110.00
• Physical and Psychological Healthcare Report – £250.00
• Solicitors/Lawyers letters/reports/questionaires – £200.00
• Fostering – AH Adult  Health Report – £73.86
• AH2 Updated report – £24.36
• Childminder Health Form – £56.25

The fees above are not an exhausted list. The surgery reserves the right to decline any request for Non-NHS services. All prices are at the discretion of the surgery and must be paid in full up front.
